PLEASE NOTE: Please submit all documents PRIOR to your appointment. If you do not submit ALL documentation, your appointment will be rescheduled.
After you have completed the intake forms, the next step is to pay for the credit report at $25.00 per person. so that we may review this report in preparation for our first meeting. You may use our convenient online payment processing form by clicking here. If you prefer not to pay online, you may mail $25.00 per applicant in a check or money order to:

After we have received your completed forms, we will call you to schedule an appointment.
PLEASE NOTE: The credit report fee is nonrefundable once we have pulled your report, which is typically the day of your appointment. If you need to cancel or reschedule your appointment, please give our office at least 24 hours’ notice to avoid unnecessarily pulling your credit report. Also note, if you submit payment and have cancelled, but do not reschedule within 60 days, the credit report fee is non refundable.
