First, Fill Out These Forms
Prior to your first meeting with us, we ask that you download, complete, and send us the forms below. When you send in your completed forms, please include $19.90 per applicant in a check or money order, so that we may pull your credit report and review it in preparation for your first meeting.
All of this information will allow us to get you the answers and help you need more quickly, and will help determine which products and services are most appropriate for your individual circumstances.
Next, Schedule an Appointment
After we have received your completed forms, we will call you to schedule an appointment.
PLEASE NOTE: In order for us to prepare to meet with you, we will pull your credit report the day before your scheduled appointment. The credit report fee is nonrefundable once we have pulled your report. If you need to cancel or reschedule your appointment, please give at least 24 hour notice to avoid us unnecessarily pulling your credit report.
